Senior Full-Stack Developer

LifeChef is building a HIPAA-compliant healthcare platform that delivers Medically Tailored Meals through complex clinical workflows, insurance billing, and third-party integrations. As a Senior Full-Stack Developer, you'll work on healthcare infrastructure serving patients, providers, and payers while building toward an agentic AI layer for clinical operations.

• Build and maintain the patient-facing experience or AI/backend integration layer (two parallel tracks)
• Develop HIPAA-compliant clinical workflows with complex state machines using XState
• Design and optimize PostgreSQL queries including window functions, CTEs, and ledger patterns for financial tracking
• Implement AWS serverless architecture using Lambda, CDK, EventBridge, and Step Functions
• Integrate with external systems: NetSuite, HubSpot, RCM billing systems, Stripe, Canvas EMR, and healthcare APIs
• Write spec-driven features using the Enso methodology with BMAD framework before implementation
• Ensure PHI/PII compliance through audit trails, secret scanning, and automated CI/CD validation
• Collaborate on agentic AI patterns and multi-step autonomous workflows

• 5+ years full-stack development with TypeScript as primary language
• Production experience with Next.js App Router (not Pages Router) and Server Components
• Deep PostgreSQL knowledge: window functions, CTEs, ledger patterns, audit trails
• AWS serverless architecture expertise: Lambda, CDK or Amplify Gen 2, event-driven patterns, IaC
• 2+ years in regulated industry (healthcare, fintech, payments) with PHI/PII handling and compliance design experience
• Daily use of AI-assisted development tools (Claude Code, Cursor, Copilot) as thinking partner
• Spec-driven development mindset with Enso methodology and BMAD framework
• Strong async communication and documentation skills
• Preferred: HIPAA compliance experience, XState expertise, agentic AI patterns, Drizzle ORM, shadcn/ui, Zod, Stripe integration depth, double-entry accounting knowledge, Vitest experience, small team experience

Team: Small, senior team, primarily Ukraine-based, no junior developers

Evaluation Process: Quick-filter questionnaire (15 min), System design conversation (60 min), AI-assisted live coding (90 min), CTO conversation (60 min), followed by 60-90 day trial sprint with defined success criteria at 30, 60, and 90 days

Development Workflow: Spec-driven development with Enso methodology, BMAD framework, Husky + lint-staged, automated PHI scanning and secret detection in CI/CD, Docker Compose for local development, GitHub Actions

Culture: Direct communication, no algorithm puzzles or whiteboard hazing, transparent about success metrics from day one
