Project Manager, Care Coordination
Summary
South Side Healthy Community Organization (SSCHO) is seeking a Project Manager, Care Coordination to develop and implement care coordination models and workflows across coalition sites. The role partners with care coordination leadership to manage workplans, milestones, and implementation activities while monitoring performance and driving process improvements.
What you'll do
Develop project workplans, milestones, timelines, and documentation to achieve SSCHO project goals.
Support project management methodologies to deliver projects on time, within budget, and to quality standards.
Maintain communication with leadership and project staff; document decisions and follow up on action items.
Create and maintain comprehensive project plans, reports, and presentation materials.
Monitor progress, manage issues, assess risks, and propose solutions.
Gather and incorporate stakeholder feedback; define requirements, scope, and objectives.
Develop and implement care coordination workflows, processes, and protocols across participating hospitals and FQHCs.
Organize and participate in stakeholder meetings; coordinate with internal and external subject matter experts.
Assess and document existing patient, clinical, and care coordination workflows at coalition sites; collaborate to customize integrations into operations.
Support technology enablement (e.g., EMR, REDCap, new tools).
Support quality improvement initiatives and ensure contractual obligations are met.
Develop and maintain key performance metrics; measure and report implementation success and provide status updates on barriers.
Contribute to evaluation and dissemination of findings and lessons learned; assist with special projects and other duties as assigned.
Requirements
Bachelor’s or master’s degree in public health, business or health care administration, non-profit management, or a related field.
2–3 years of project management experience.
PMP certification encouraged (not required).
Preferred: 2–3 years overseeing population health management, care management, similar programs, or related healthcare consulting.
Experience with health systems, FQHCs, CBOs, and/or collaboratives; experience planning/implementing/evaluating community-based interventions.
Non-profit management experience and knowledge of NCQA, Joint Commission, and state/federal agency requirements.
Strong business acumen; sound judgment, troubleshooting, and problem-solving skills; ability to produce polished deliverables.
Ability to build credibility, manage relationships, resolve conflict, and communicate effectively in writing and presentations.
Ability to manage change and adapt as project needs evolve; commitment to racial and health equity; hands-on execution mindset.
Knowledge of quality improvement, data collection, and data management.
Proficiency with Windows-based systems and tools such as EMRs (e.g., EPIC) and Microsoft Office (Excel, Visio, Outlook, Word, PowerPoint); ability to learn new applications and vendor software for reporting.
Ability to maintain strict confidentiality in compliance with HIPAA.
Conditions
Hybrid work schedule.
Local travel to South Side of Chicago healthcare organizations (safety net hospitals, health systems, and Federally Qualified Health Centers) may be required.
General office environment with moderate noise level.
Must complete company trainings as assigned and support organizational mission/values.
