Dario Health is a digital health company on a mission to make better health easy for members worldwide. The Clinical Health Coach supports and educates members with chronic conditions, helping them set goals and improve health outcomes using Dario’s coaching framework and platforms.
What you'll do
Manage a panel of high-risk members to prevent and/or manage chronic conditions using the Dario Way coaching framework and help members set health goals.
Build relationships with members through the Dario app, email, telephone, and electronic messaging.
Support members via the Dario Engage platform by responding to clinical triggers and member activities.
Proactively follow up with members missing key measurements or struggling to keep metrics within normal ranges.
Assist in developing clinical content for members, clients, and internal staff.
Requirements
Registered Dietitian (RD) certification (required).
Bachelor’s degree or higher.
5+ years of experience in health coaching, health education, nutrition, or behavioral health.
Experience with chronic disease management education (diabetes experience is a plus).
Familiarity with motivational interviewing and behavior change theories.
Open-minded, non-judgmental, compassionate coaching style.
Health Coach certification (preferred).
Bilingual English/Spanish (preferred).
Strong organizational, communication, and time management skills; results-oriented.
Independent, flexible self-starter comfortable with ambiguity and a fast-paced environment.
Ability to learn, analyze, understand, and convey complex information.
Demonstrated technical aptitude and proficiency with technology and web-based applications.
