Patient Care Coordinator
Summary

The Patient Care Coordinator partners with a multidisciplinary care team to communicate and document high-risk patient profiles and connect patients to appropriate follow-up care and resources. The role supports coordinated care delivery by closing care gaps and aligning care with patients’ health and lifestyle goals.
What you'll do
Outreach to and engage high-risk and post-discharge patients needing additional care management support, ensuring PCP follow-up appointments are scheduled and care gaps are closed.
Collaborate with care team members and external vendors to support patient needs (e.g., durable medical equipment and home health services).
Request and coordinate medical records from physicians and healthcare systems to ensure continuity of care for PCPs and the interdisciplinary team.
Screen patients and escalate to nursing and social work team members based on individual needs.
Assist nursing and social work team members with patient follow-up and provide tools and resources as directed.
Monitor stable patient populations as directed by nursing and social work team members.
Address open care gaps with patients and ensure adequate supporting documentation is available in the medical record.
Coordinate with local community resources to support physical and psychosocial patient needs.
Collaborate with multidisciplinary team members to improve patient outcomes and deliver best-in-class care.
Participate in special initiatives as directed by market leaders.
Requirements
2+ years of experience as a Licensed Practical Nurse (LPN) or state equivalent, or as a Medical Assistant (MA).
Ability to manage multiple tasks and maintain an organized, systematic workflow.
Flexibility in ambiguous and dynamic environments with a positive, proactive attitude.
Strong collaborative problem-solving skills and willingness to learn independently and take initiative.
Ability to engage diverse populations with cultural sensitivity.
Honesty, follow-through on commitments, and strong communication to build trust.
Ability to problem-solve quickly and identify issues requiring escalation.
Comfort with technology, including Microsoft suite products.
Prior experience using electronic health records (EHR), including data capture, data mining, and reporting.
