Summary
Privia Health is a national physician platform focused on improving healthcare delivery through physician groups, accountable care organizations, and population health programs. The Health Advisor provides telephonic care coordination and support for at-risk patients, helping close care gaps and improve outcomes through follow-up, coordination, and education.
What you'll do
Conduct timely post-discharge follow-up calls after inpatient visits to assess needs and reinforce the plan of care.
Review and reconcile discharge medications and escalate concerns to the Primary Care Provider (PCP).
Coordinate and/or schedule follow-up appointments with PCPs and specialists.
Identify high-risk patients and refer them to the appropriate Care Management team.
Review Emergency Department discharges and facilitate appropriate follow-up care.
Identify and address Social Determinants of Care needs by connecting patients with appropriate resources.
Assist in review and completion of open quality gaps.
Provide inbound and outbound telephonic support to the care management team.
Perform other duties as assigned while maintaining HIPAA compliance.
Requirements
Certification as an Emergency Medical Technician (EMT) or Medical Assistant (MA).
Preferred 3–5 years of relevant experience.
Prior experience in an outpatient healthcare setting (strongly preferred).
Experience using electronic medical records (EMR) systems.
Proficiency with basic technology platforms and ability to navigate multiple systems.
Strong foundational medical knowledge (common disease states, diagnoses, medications, care plans) and critical thinking skills.
Strong attention to detail and ability to manage multiple tasks and priorities.
Strong communication skills with the ability to engage patients effectively over the phone.
Must comply with all HIPAA rules and regulations.
Conditions
Compensation: $20.00–$23.80 per hour base pay, plus annual bonus targeted at 10%.

Remote technical requirement (if remote): minimum 5 Mbps download and 3 Mbps upload internet speed; home office workers may be eligible for expense reimbursement to offset internet cost.
