As an SMS Care Advocate, you'll be the primary contact for customers navigating healthcare, delivering personalized support through phone and coordinating care across multiple settings to improve outcomes and reduce costs.
What you'll do
Make outbound calls to inform members of benefits and guide them through care decisions; receive inbound calls to educate members and schedule appointments; coordinate care across various settings while maintaining confidentiality; update systems to document member engagement and appointment information; listen to and relay member feedback internally; collaborate with departments and insurance partners; address member inquiries and complaints; achieve performance metrics and targets; participate in ongoing training.
Requirements
High school diploma required (Associate's or Bachelor's degree preferred); 1-3 years customer service, service center, or sales enablement experience; knowledge of care coordination and case management preferred; strong written and verbal communication skills; critical thinking and problem-solving abilities; ability to organize work and meet deadlines; bilingual Spanish and English preferred.
Conditions
