Quality Control Specialist - Health Home

Urban Health Plan, Inc. is seeking a Quality Control Specialist to support its Health Home program serving Medicaid patients with chronic or qualifying conditions. The role focuses on ensuring compliant documentation, maintaining data integrity, and enabling accurate, timely billing through quality checks and audits.

• Review Health Home billing and resolve errors or discrepancies.

• Perform quality assurance checks on documentation, Health Home documents, and services.

• Maintain and update Excel trackers for program data and performance metrics.

• Develop reports to track trends, performance, and gaps.

• Conduct quarterly chart audits to ensure accuracy of documentation and services.

• Assist with quality assurance initiatives.

• Provide administrative support such as data entry, filing, and scanning, and other duties as assigned.

• High school diploma required (advanced education or certifications in billing, Excel, or health information management are a plus).

• 1–2 years of experience with billing, reporting, medical records, and/or tracking.

• Advanced proficiency with Microsoft Office, especially Microsoft Excel.

• Bilingual English/Spanish required.

• Intermittent hybrid: primary duties are remote with a minimum of 2–4 onsite days per month.

• Onsite work located at Bronx offices.
